Required Registration Documents

We strongly suggest uploading your documents during online registration. You may also email them or drop them off at your school or the district’s Central Office.

All documents MUST be on file before your student can attend school.

Proof of Residency
- Residency means where you truly live. Residency requirements are not merely met because one owns property, has a business, or has a post office box in South Burlington.
- Please review the full list of accepted documents on the Required Registration Documents page.
Birth Verification with Proof of Age (a copy is okay)
- Birth certificate or
- A valid, unexpired Passport Birth Certificate, or
- A valid, unexpired Passport
Legal Documents (if needed)
- Custody papers
- Protection orders that include the student
Health Documents* (Required for in-house prekindergarten through Grade 12 only)
- Immunization Record -
  Must meet Vermont Department of Health requirements
- Well-Care / Health Examination Form -
  Required for Prekindergarten students at Chamberlin Preschool, Orchard Preschool, and RMCS Preschool

* Health documents are not required for Act 166 registration.
